Most often, it is the children of a senior who seek the services and professional counseling of a geriatric care manager. They may be separated from the parent geographically or are not available as much as they would like. They need help navigating the issues surrounding their parent's care. Trust officers, attorneys, physicians, and other health professionals also turn to Boutique Care Management and often refer families and seniors to us.
The first step will be to meet and discuss our services. We gain some insight regarding the prospective client and his or her family, and talk about the possibility of moving forward to a full assessment. After this initial no-cost meeting, we develop and implement the care management plan within a few days, depending on the
individual situation.
Once you decide to let Boutique Care Management serve you, we do a comprehensive assessment. Your nurse care manager meets with the senior, along with the family or designee, to understand the individual and his or her environment. She conducts a physical exam, assesses medical records, reviews medications, and evaluates the care setting, including any possible safety concerns, caregivers and others with
whom the senior interacts with on a daily basis. Other possible support providers,
such as additional or alternative caregivers, Meals on Wheels, and physical therapy
are discussed.
When we have a clear picture of the senior's daily life and medical needs, the nurse care manager develops a comprehensive plan that honors the individual's health wishes and works to improve his or her quality of life. The plan is then presented to the client and family.
When the plan is approved implementation begins. Your nurse care manager makes modifications to established care, which often includes changes to medication systems, physical space configuration, physician consultations and establishing assistance from in-home caregivers. The nurse care manager then oversees the senior's needs and visits the client at least once a week to oversee the execution of the plan and coordinate and direct caregivers.
