Conduct a comprehensive geriatric and home safety assessment
Develop a geriatric plan of care
Visit the home or care setting routinely
Establish a personal relationship with the senior
Provide round-the-clock support wherever we're needed
Assist with insurance and Medicare issues
Manage medications
Provide outside referrals for at-home skilled nursing care
Train and supervise in-home staff
Supervise hospital and nursing home care
Coordinate medical services and care
Accompany the senior to physician visits or outpatient procedures
Serve as liaison between the client and medical, social service and
psychiatric professionals
Provide referrals for professional therapeutic counseling services
Facilitate hospital or nursing home discharge

For our clients, we're available 24 hours a day, 7 days a week, so you can call us with questions or requests whenever you need us.
